Complete applications are due Thursday, April 9, 2009 by 4:00 p.m. at the following address: Big Sky CFC, 2920 2nd Ave. North, Billings, MT 59101

1. All applying agencies read Attachment A carefully.
2. Organizations that are part of an IRS group exemption and bona-fide chapters/affiliates of a national organization, please read Item 2 closely.
3. Current determination letters from the IRS can be requested by calling the IRS at (877)829-5500.
4. Changes have been highlighted for your convenience in instruction and certification numbers 6 and 7 of the model 2009 Local Independent Organizations and Members of Federations Application and instruction and certification numbers 5 and 6 of the model 2009 Local Federation Application regarding Form 990
